RHINOPLASTY IN IRAN: ALL YOU WANT TO KNOW

Nose job is the most popular cosmetic surgical procedure in Iran. Iran is known as the ‘rhinoplasty capital of the world’.
More than 150,000 rhinoplasties in Iran are being performed annually. This number of surgeries has led Iran to have the most experienced doctors for rhinoplasty. In addition, the advanced methods of rhinoplasty, subspecialty hospitals, and post-operative care have significantly facilitated the rhinoplasty in Iran.
WHAT IS RHINOPLASTY?
Rhinoplasty, commonly referred to as a “nose job,” is a surgery to change the shape of your nose by modifying the bone or cartilage. Rhinoplasty is one of the most common types of plastic surgery. People also get rhinoplasty to repair their nose after an injury, to correct breathing problems or a birth defect.
HOW IS A RHINOPLASTY PERFORMED?
A nose job is usually done as an outpatient procedure, meaning there is no overnight stay. You will get general or local anesthesia. With general anesthesia, you will sleep throughout the operation. With local anesthesia, you will be sedated and your nose will be numbed so you are relaxed and unable to feel the pain.
During an operation, the surgeon makes cuts within the nostrils. In difficult cases, the surgeon may also make cuts across the base of the nose. The surgeon then reshapes the inner bone and cartilage to produce a more pleasing appearance.
Rhinoplasty for medical reason for example, if the septum –– the mid line wall of cartilage that divides the right and left sides of the nose –– is deviated, then a septoplasty alone may fix the problem. However, when a septal deviation is more severe, or occurs nears certain critical areas of nasal support, a rhinoplasty is required to ensure proper breathing and nasal shape. Rhinoplasty (also called septorhinoplasty) addresses the septal deformity and strengthens key breathing areas of the nose with strategically placed cartilage grafts.
IS RHINOPLASTY A RIGHT CHOICE FOR ME?
Rhinoplasty can be done for medical reasons. A common medical reason for rhinoplasty is difficulty breathing through the nose. Nasal obstruction can cause problems with exercise, disturb sleep, contribute to snoring and sleep apnea, or interfere with other activities. If medical treatments (such as nasal spray or sleep apnea treatment) fail, surgery may be the next step.
Rhinoplasty for cosmetic reasons in recent years is also very prevalent. The most common requests for cosmetic nose operation are reducing a nasal hump, refining the tip, or correcting asymmetries.

TYPES OF NOSES THAT GO UNDER SURGERY
In rhinoplasty terms, there are two types of noses that go under surgery: bony nose and fleshy nose (meaty nose). The difference between them lies in the thickness of skin. Bony noses are mostly thin or medium-thick. However, meaty noses are thick.
Structures of bony noses are clearly noticeable because of their thin skin. Bony noses have mostly strong cartilage with firmer nasal tip and less droopiness.
Instead, fleshy noses for their thick skin do not show structure and cartilage details. In addition, the fleshy noses have weaker and thinner cartilage tissues.
Occasionally, some noses might not fall under the above category. Sometimes bony noses might have thick skin and some fleshy noses might have a thin skin with strong cartilages.
COMMONLY KNOWN NOSE JOB STYLES AMONG IRANIANS
There are three styles of nose job performed by Iranian doctors and recognized by all Iranian patients. Fantasy (or dolly), semi-fantasy, or natural.
NATURAL NOSE JOB
Natural rhinoplasty style has smaller changes to the nose, a straight nasal bridge instead of a curved one. In addition, the angle between the nasal tip and the upper lip is not too wide in a natural nose job style.
‘NATURAL’ STYLE FOR BONY NOSES
Natural style for Bony noses with dorsal humps can be good choice.
NATURAL STYLE FOR FLESHY NOSES.
Fleshy noses usually get natural-style nose surgery.
FANTASY NOSE JOB (DOLLY NOSE)
Fantasy nose job is a very small and upturned nose (casually named as ‘ski jump’ or ‘snub’ nose). Fantasy nose is made small and upturned. It produces a favorable result mostly in bony noses and most attractive in small faces.
SEMI-FANTASY NOSE JOB
A semi-fantasy nose job style lies between the natural style and the fantasy style. It has slighter curve of the bridge and the angle between the nose and the lips is narrower than a fantasy nose.
Fantasy and semi-fantasy noses style are more favorable among women. Men more likely tend to have natural nose job style.
NOSE JOB FOR MEN (MALE RHINOPLASTY)
Today’s men also have more tendency to get rhinoplasty. However, there are some difference between male and female rhinoplasty.
Due to different structure and features of men’s nose, which have straighter and wider bridge, thicker skin, and the tips are often more bulbous and broader, so the result of nose job for men also very different. Men tend to have natural nose job style.
WHAT HAPPENS BEFORE, DURING AND AFTER YOUR RHINOPLASTY?
Being worried before your operation is completely normal. However, the more you learn about the procedure, the less concerned you will be and more prepared for your surgery.
Going abroad for your procedure makes you even more concerned. You will have to go through all travel preparations of getting visa, booking hotel and flight, transfer and so on…
Teratona will help you by taking your concerns way with handling all the travel services. You will not have to be worried about your travel plan to Iran. All you need to do is to be optimistic and be prepared for your surgery.
During your nose operation, you will go under general anesthesia so you will be unconscious and do not feel any pain. After your surgery, a nasal splint is put on your nose to help nasal structure remain stable during the healing process.
Nose surgery is an outpatient procedure, which means you will not have to stay the night at the hospital. Five to seven days after surgery, you need to visit your surgeon to get your nasal splint removed and the bandage replaced. During this period, you have to follow your doctor’s instructions for better recovery.
HOW DO YOU CHOOSE THE RIGHT PLASTIC SURGEON?
It is important to remember that you will be trusting this person with your appearance, so you’ll want to make this decision carefully.
Choose a surgeon who is board-certified. Therefore, you can trust that your surgeon is qualified.
Review his or her before and after photos of their works. This will give you an idea of how well he or she will be able to achieve the look you are hoping for. It can also be helpful to bring photographs of the look you want to achieve.
In addition, meet face-to-face with any surgeon you’re considering. You should feel comfortable with your surgeon, and he or she should be able to confidently answer any questions you have.
POST-OPERATIVE FREQUENTLY ASKED QUESTIONS
WILL I SEE RESULTS RIGHT AWAY?
Most patients see initial results after about a month. However, your final nose shape may not be completely visible for up to a year after surgery.
WHEN CAN I GO BACK TO MY DAILY ROUTINE?
This varies from one patient to the next, but most people are able to return to work and light activity after about three weeks. Those with strenuous jobs may need a longer recovery period before they are cleared to return to work.
WHAT TO EXPECT DURING RHINOPLASTY RECOVERY
First, it’s important to understand that it can take up to a year for the swelling to completely subside and the final shape of your new nose to emerge. Most patients will see a gradual change in appearance over the first few weeks as the tissues heal, and the swelling continues to disappear.
The soft tissues of the nose tend to hold swelling longer than most other places on the body, so even though you will see a marked difference after three to four weeks, your final nose contour may take several months to appear.
For most rhinoplasty patients, the surgical site will be packed or splinted on the inside, and the nose will be covered with bandages on the outside to provide protection and support to your new nose shape. This dressing will typically need to be kept in place for one week, and your surgeon will then remove the dressing and any stitches.
Pain and swelling will be the most pronounced in the first week post-surgery, and you will likely have some bruising and tenderness on your cheeks and under your eyes for a week or two, but this typically heals.
In addition, your surgeon will give you specific and detailed post-operative instructions. This will include post-op care, medication schedules and concerns to monitor for. Following these directions will give you the best possible outcome.
REVISION RHINOPLASTY
UNDERSTANDING AND WHY PATIENTS SEEK OUT THIS PROCEDURE
Most plastic surgeons agree that rhinoplasty is one of the most challenging cosmetic (aesthetic) plastic surgery procedures for many reasons. The nose is a complex structure with an obvious position in the middle of one’s face. Expert rhinoplasty requires simultaneously correcting nasal deformities while preserving maximal nasal function. One cannot make a nose look good without optimizing nasal airway function.
WHY DO PATIENTS CHOOSE REVISION RHINOPLASTY?
Patients who are looking for a revision or secondary rhinoplasty challenges the skills and experience of plastic surgeons. Revision rhinoplasty applies to any patient who has previously undergone rhinoplasty one or more times and desires improvement in the appearance and often the function of the nose. These are among the most difficult cases aesthetic plastic surgeons face for several reasons.
For one, patients are often unhappy with their prior surgery and may not understand that further rhinoplasty may not be successful in completely correcting cosmetic deformities which were not corrected in the last surgery or occurred as a result of surgery. Scar tissue from prior rhinoplasty is often an issue in revision cases and can limit the final outcome since it may recur even after successful revision rhinoplasty. Also, revision rhinoplasty often requires the use of cartilage to replace damaged and/or deficient cartilage removed in the prior surgery.
The nasal septum may be deviated, which requires correction to improve both the appearance of the nose and its function. Nasal obstruction may also be caused by a collapse of the tip cartilages resulting in external valve collapse. Deviation of the nasal septum along with deficiencies in upper lateral cartilages are a common occurrence in revision cases resulting in internal valve collapse requiring the use of cartilage grafts called “spreader grafts” to spread open this narrowed area.
WHAT IS DONE IN A REVISION RHINOPLASTY?
Revision rhinoplasty often requires cartilage for structural support for the tip and/or bridge of the nose. Septal cartilage is often deficient and plastic surgeons often must choose cartilage from elsewhere in the body, particularly the ear and occasionally the rib. Soft tissue irregularities along the bridge of the nose or tip may also require tissue that provides a cover or camouflage and acts as a “natural filler” which is permanent. An excellent source is “temporalis fascia,” which can be taken from a remote incision within the hairline and results in no noticeable deficit. Occasionally, scarring within the nose requires the use of skin grafts to reopen a distorted and narrowed nasal airway.
Despite a surgeon’s best efforts, revision rhinoplasty has a higher revision rate than primary rhinoplasty (no prior nasal surgery). It is important that you are aware of this as no ethical plastic surgeon can guarantee the outcome of revision rhinoplasty or any other cosmetic plastic surgical procedure for that matter.
If you are considering revision rhinoplasty then make sure your board-certified plastic surgeon has substantial experience with rhinoplasty. Experienced rhinoplasty surgeons are more likely to see revision cases than those who do this operation infrequently. Look at your prospective surgeon’s website to see how many rhinoplasty cases are in their photo gallery. Computer imaging may be helpful for your surgeon to visually communicate what potential outcome you may expect from surgery. Previous rhinoplasty patients are also a great resource for prospective patients; your plastic surgeon should be able to supply you with patients who have offered to speak with you and others and discuss their experience in some detail.
In the end, your decision to undergo revision rhinoplasty is a personal one. Choosing an experienced, board-certified plastic surgeon who has “connected” with you and earned your trust will offer the best chance of a successful outcome. Most patients who are unhappy with the outcome of primary rhinoplasty are pleased with the final result from revision rhinoplasty if they follow these simple recommendations.
RECOVERY
You may be eager to see the results of your rhinoplasty procedure right away, but the recovery process takes time. During this time, there are steps you can take to speed things up and ensure that everything heals properly.
RHINOPLASTY RECOVERY TIMELINE
The average rhinoplasty recovery time is one year. That may sound like a long time, but it becomes much more manageable when it’s broken down into a timeline. The following recovery milestones may vary from patient to patient, but generally, it goes something like this:
1 -Week: The splint can be removed from your nose and you can go out in public without showing noticeable signs of surgery unless there is bruising around the eyes. This may take 2 weeks to resolve. It is safe to resume activities of daily living.
2 Weeks: The majority of the facial swelling has subsided and most of the bruising should have resolved.
3-4 Weeks: You can safely return to cardiovascular activities such as jogging, swimming and cycling.
6 Weeks: The bones are stable, you can resume resistance workouts (weight lifting), wearing glasses and blowing your nose.
3-6 months: The numbness and abnormal sensations in your nose and nasal skin should be resolved.
1 Year: The healing process is complete – swelling should have subsided entirely and the nose’s new shape is fully refined.
Is there any way to speed up this timeline? There are actually quite a few things you can do to make your plastic surgery recovery go by more smoothly. We recommend the following tips:
1. LISTEN TO YOUR DOCTOR
First and foremost, follow your doctor’s instructions. You will receive specific instructions on what medications to take, when to take them, how to care for the surgical site to prevent infection, and when you’ll need to come back in for a follow-up exam.
2. LISTEN TO YOUR BODY
Your plastic surgeon will be able to see how your rhinoplasty recovery is going from the outside, but only you will know how you’re feeling. Listen to your body – if something doesn’t feel right after surgery, discuss it with your doctor right away.
3. KEEP YOUR HEAD ELEVATED
Sleeping on your side after rhinoplasty isn’t just uncomfortable – it can prolong your recovery time by causing additional bruising and swelling. More frighteningly, it can displace your nose. To avoid this, you’ll need to keep your head elevated at night for 6 weeks following surgery. One of the easiest ways to do this is to prop your head up on two or three pillows or use a foam wedge. Sleeping in a recliner also works well.
If you tend to toss and turn, you can keep your head in place with a travel pillow or by surrounding yourself with rolled up towels.
4. USE COLD COMPRESSES
In the first 72 hours after surgery, using a cold compress can help reduce swelling. Just be careful not to apply the compress directly to your nose. Instead, apply it to your cheeks so that you don’t accidentally shift any bones or cartilage.
5. GET PLENTY OF REST
The congestion you feel after surgery may make it more difficult to fall asleep, but rest is a vital part of the recovery process. Your body uses energy to heal itself, and getting a solid seven or eight hours of sleep, each night gives your body time to restore that energy.
6. MAINTAIN A HEALTHY DIET
What does eating your vegetables have to do with recovering from a nose job? A lot, actually. Eating a balanced diet post-surgery can help to speed up your recovery by giving your body the nutrients it needs to heal itself.
Protein is an essential building block of skin, muscle, cartilage, and blood. A diet consisting of nuts, meat, eggs, yogurt, fish, and other protein-rich foods will help your body rebuild the damaged tissue in and around your nose.
You’ll also want to make sure you’re getting plenty of vitamins in your diet, namely Vitamin A and Vitamin C. Vitamin A, which is commonly found in dark, leafy greens like spinach and kale, It is a great immune booster that can help you fight off infections after surgery. Vitamin C plays an important role in the regeneration of collagen, the protein that connects your skin tissue. Good sources of Vitamin C are citrus fruits, Brussels sprouts, and broccoli.
7. STAY COOL
Steamy showers, hot bowls of soup, and relaxing saunas might feel good, but they’re not so good for your swollen nose. Heat will cause the tissues in your nose to swell even more, which is why it’s best to stick to a diet of cool and room-temperature foods, take lukewarm showers, and stay out of the heat in the weeks after your nose reshaping procedure.
8. DON’T BLOW YOUR NOSE
After surgery, you’re going to experience some congestion for a few weeks, maybe even a few months. This feeling is caused by swollen nasal tissues. Resist the urge to blow your nose and talk with your doctor about using a saline nasal spray to gently moisturize your nasal passages until the swelling goes down. You can resume blowing your nose after 6 weeks from surgery.
What about sneezing? You can’t just choose not to sneeze. Instead, try to sneeze through your mouth instead of your nose. It sounds gross, but it’s better than damaging your fragile nasal passages and slowing down your rhinoplasty recovery time.
9. AVOID VIGOROUS EXERCISE
In most patients, it takes roughly 6 weeks for the bones in your nose to heal following surgery. During this time, you should avoid strenuous exercise. Even movements that seem harmless like stretching, lifting, or bending over can increase nasal swelling. Wait until your doctor gives you the green light to resume normal activity before you begin to ease back into your workout routine.
10. DON’T WEAR GLASSES
Sunglasses, reading glasses, prescription glasses – anything that puts pressure on your nose as it’s healing can cause additional bruising, swelling, or even indentations that will require revision rhinoplasty down the road.
Your facial plastic surgeon will advise you on when it’s safe to start wearing glasses again. In the meantime, switch to contacts, if you can. If you can’t, wear the lightest possible frames, and only wear them when you absolutely need to.
11. STAY OUT OF THE SUN
It’s always a good idea to protect your skin from getting too much sun exposure, but this is especially important after a nose job. Not only is your nose more prone to sunburn in the weeks and months following the procedure, but too much sun can also cause any scarring you have to darken and swelling to increase.
12. DON’T SMOKE
Both before and after surgery, it’s important that you don’t smoke and avoid secondhand smoke if possible. The nicotine in tobacco products limits blood flow, making it more difficult for your body to heal itself and interferes with your plastic surgery recovery.
13. BE PATIENT!
Above all, have patience. You will heal, and you will have the rhinoplasty results you want – it just takes time. Following the steps above won’t turn a year-long recovery process into a week-long one, but it will help speed things up and improve your post-surgery experience.
HOW SHOULD I PREPARE FOR RHINOPLASTY?
In preparing for rhinoplasty surgery, you may be asked to:
Get a lab test
Take certain medications or adjust your current medications
Stop smoking
Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding
Your plastic surgeon will also discuss where your procedure will be performed.
Be sure to arrange for someone to drive you to and from surgery clinic or hospital.
WHAT SHOULD I EXPECT FOR RECOVERY?
You should expect to take one week off from work. During that time rest, ice, and a regimen of nasal cleaning is recommended. Also pain medications are prescribed by your doctor.
During recovery, it is difficult to easily breathe through the nose because splints (bendable, thin plastic sheets) are placed inside the nose to keep the septum straight. Swelling peaks on the third day, and then decreases. Bruising under the eyes can sometimes occur. During the week after surgery, you will wear a small cast on your nose to protect it and reduce swelling.
after a week of surgery, the splints and cast are removed in the doctor’s office. You may then return to work and resume cardio exercise, such as walking or running. You will need to avoid contact sports for six weeks. Otherwise, within two weeks after surgery, you should be back to your normal activities and routines.
While you are likely to heal quickly during the first month, full healing may take up to one year. The majority of swelling
