LASIK EYE SURGERY IN IRAN: ALL YOU NEED TO KNOW

WHAT IS LASIK EYE SURGERY?
LASIK, which stands for laser in-situ keratomileusis, is a type of laser light surgery on the eye, typically performed to treat myopia, hyperopia and astigmatism, and generally to permanently remove glasses. LASIK surgery allows you to put aside your glasses or contact lenses and get rid of their limitations. LASIK, like other types of vision treatment surgery, changes the shape of the cornea and does so with a laser beam and corrects the cornea so that when light enters the eye, it can focus on the retina properly.
In most cases, LASIK surgery is painless and does not require general anesthesia and is performed on both eyes within 15 minutes. The result is improved vision without the use of glasses or contact lenses, and usually within 24 hours, the first results appear.
If you are not eligible for this procedure due to the thinness of the cornea, there are other types of vision correction surgeries such as PRK, LASEK and IOL. Your doctor will determine which of these methods is best for your condition. If you cannot use laser surgery due to keratoconus, you can use various new methods such as scleral lenses to treat your keratoconus.

HOW IS LASIK PERFORMED?
LASIK is a very simple operation and is an outpatient procedure. You can do the whole procedure – from a thorough corneal examination to the operation itself – in one day.
Prior to surgery, anesthetic drops are injected into the eye several times to prevent any discomfort during the surgical procedure. Doctor may also prescribe sedatives to relieve patients with severe stress. Then, when the eye is exposed to the laser beam, a device called eye speculum is used to hold the eyelid open.

CANDIDATES FOR LASIK SURGERY
All people with mild to moderate myopia, hyperopia or astigmatism who have sufficient corneal thickness.

THE REQUIRMENTS OF LASIK EYE SURGERY
Minimum age is 18 and maximum 55 as long as the person does not have cataracts.
No significant change in the score of glasses in the past year, i.e. a maximum of more than 5.0 diopters have not changed.
Lack of severe Keratoconus, presbyopia, cataracts, diabetes and severe dry eyes
No pregnancy and lactation for at least the first 3 months
Sufficient thickness of cornea
Myopia between -1 to – 10 Diopter of per eye
Astigmatism 5.0 to 6 diopters
Hyperopia maximum 6 diopters per eye.

LASIK SURGERY FOR ASTIGMATISM, MYOPIA AND HYPEROPIA
The Food and Drug Administration (FDA) has approved the excimer laser for correcting myopia up to -12 diopters with -4 diopter astigmatism, and hyperopia from +1 to +6 with 1 diopter astigmatism. (Diopter is a measure of refractive error. A negative diopter is myopia and a positive diopter is hyperopia).
Anyone who is undergoing eye surgery wants to know what the chances are of getting a satisfactory result in their operation? For low and moderate myopia (up to -7 diopters), about 95% of patients achieve spectacle-free vision of 10.5 to 10.6 or more; and they have an 85% chance of seeing without glasses 10.8 to 10.9 or better. This means that people’s dependence on glasses will be greatly reduced. However, for more precise eye work they may need glasses with small number – such as driving – especially at night.
The idea that LASIK cannot treat astigmatism is wrong. Normally astigmatism does not rise above 3 diopters and can be completely corrected by LASIK. Even greater amounts of astigmatism can be treated with LASIK ; However, it is possible that second LASIK surgery may sometimes be needed for very high scores.

PREOPERATIVE MEASURES OF LASIK EYE SURGERY
A complete eye examination is performed before surgery to ensure eye health. When this is done, the doctor measures the curvature of the cornea and the size and position of the pupils, the shape of the eyes and the thickness of the cornea. All of these procedures are performed so that the surgeon can be sure that the cornea has enough tissue for the operation and vision recovery will not face a problem. Because a thin cornea can be problematic for laser eye surgery.
Eye moisture will also be assessed and preventive recommendations may be given to patients to reduce the risk of postoperative dry eye. Usually a tool called a corneal topography is used to measure the curvature of the surface in front of the eye and create a design of the cornea.
Your doctor will also check your general eye health history and any medications you are currently taking to determine if you are eligible for LASIK. You should stop using lens for the period of time prescribed by your doctor (usually about two weeks) before the eye exam and Lasik surgery; Because the use of lenses can change the natural shape of the cornea.

WHAT HAPPENS DURING LASIK SURGERY
The LASIK surgeon marks the cornea before creating a flap. A suction ring is placed over your eye to prevent eye movements, which can affect the quality of the flap.
The LASIK surgeon first uses a surgical instrument called a microkeratome to make a thin circular incision (flap) in the cornea. (To make the cut in the operation of Femtolasik and femtoesmile , the femtosecond laser used by very advanced devices). After creating the corneal flap, the surgeon uses computer to adjust the excimer laser to the patient’s specified score. The surgeon then opens the flap to access the cornea to remove some of the corneal tissue using an excimer laser.
The doctor, while watching your eyes with a special microscope, will ask you to star at a specific area, the source of a green laser light, for a short time. The laser sends light pulses to your cornea. This highly specialized laser uses ultraviolet light to alter the shape of the cornea by removing microscopic amounts of tissue to improve vision by helping light focus on retina more accurately.
Pulses of laser light change the shape of the surface of the cornea without causing pain, burning or bleeding. However, there may be a slight pressure on the eye. While the laser is working, you will hear a clicking sound on the surface of your eye. LASIK is performed on each eye separately, and surgery will take only about 20 minutes for both eyes.

DURATION OF LASER EYE SURGERY
Procedure time: about 10 minutes per eye
Results: 10/10 vision without the need for glasses or lenses
Recovery time: a few days to a few weeks to stabilize vision

IMMEDIATELY AFTER LASIK SURGERY
Immediately after LASIK surgery, there may be a burning, itching, or foreign body sensation in the eye. There may be a slight feeling of discomfort for which the surgeon may prescribe a mild analgesic. It is very important that the eye not be “rubbed” after LASIK so that the flap does not move in the eye.
On the same day of surgery, vision remains blurred or foggy; But the next day the vision improves. The doctor should be notified immediately if severe pain occurs or any of the symptoms worsen before the follow-up visit (which is 24 to 48 hours after surgery).

LASIK POSTOPERATIVE EXAMINATIONS
At the first visit to the doctor’s after LASIK surgery (usually 48 or 72 hours later), the protective lens is removed and the doctor examines your eye and your vision. You may receive eye drops to prevent infection. For added safety, avoid driving until your vision is fully recovered.

DO’S AND DON’TS OF AFTER LASIK SURGERY
Driving is the first avoidance after Lasik surgery.
Avoid bathing for 3 days with your head under water shower.
Avoid swimming and Jacuzzi for up to 3 weeks.
Avoid strenuous exercise and lifting heavy objects for up to 2 months.
Avoid medical solutions, creams and eye cosmetics for 2 to 4 weeks.
Do not rub your eyes for a month.
Use the prescribed drops according to the doctor’s instructions.
Wear sunglasses in bright light and outdoors.
You will be given more detailed instructions by your doctor. It should be noted again: you cannot drive after LASIK, and the doctor must examine your eyes the day after surgery and confirm your vision according to the legal standard for driving.

RESULTS OF LASIK EYE SURGERY
Laser eye surgery offers many benefits and can dramatically improve your quality of life. The colors have become very clear to you and you will have a lot of pleasure in seeing things clearly. Most people will achieve 10/10 vision or more after surgery; But the results of this operation are different in people.
You may still need to wear glasses or contact lenses after laser surgery; However, your eye score will usually be much lower than before. If you have mild refractive errors after LASIK and need more accurate vision for certain activities, such as driving at night, prescription lenses with anti-reflex coating can often be useful. If your eyes are sensitive to sunlight after the operation, you can use photochromic eye care glasses.

NOTES WHEN CONSIDERING LASIK SURGERY:
Choose an experienced eye surgeon. Make sure he or she has done a lot of LASIK procedures before and whether he or she is using advanced equipment and new technologies.
Talk to your doctor about your health issues and medications you are taking.
Be sufficiently aware of the relevant issues. Are you aware of the available methods of LASIK surgery? And do you know the cost of your surgery?
Are you aware of possible complications and limitations after surgery?
Be realistic! Do you have realistic expectations of what LASIK surgery can do for you?

AFTER CARE LASIK EYE SURGERY
Although different surgeons may have different recommendations, here are the usual advices after LASIK surgery for daily activates.
Driving: You cannot drive after the LASIK operation and the next day when you come to revisit your doctor, someone must take you there. Your doctor will recheck your vision and will tell you if you are legally allowed to drive without lenses or glasses.
On the day after the operation, taking a shower or bath will not be a problem; But to prevent soap or water from entering your eyes, you need to be careful for at least a week. In general, if your work environment is clean, you can return to work in a day or two. Often after LASIK surgery, you should use artificial tears for at least a week to reduce the risk of dry eye, especially if you spend a lot of time in front of a computer screen.Using eye makeup: It is better not to use eye makeup for a week. Also, it is better to throw out your old false eyelashes and buy new ones (to avoid the risk of using false eyelashes that contain infected bacteria and can cause eye infections). It is better to observe these measures for other eye makeup items as well.
Exercise: You can resume your exercise in a few days. But do not let the sweat of your body reach your eyes and avoid rubbing your eyes.
Swimming and hot bath: Wait at least two weeks after LASIK surgery.
Note: the above are general guidelines only. Consult your doctor or surgeon for more detailed information on whether you can perform your normal activities after LASIK.

RISKS AND COMPLICATIONS OF LASIK
LASIK and other laser eye surgery, such as PRK and LASEK have an excellent safety record and success rates are very high. They are designed to treat myopia , hyperopia and astigmatism and can treat your eyes without the need for glasses or contact lenses.
Dangerous and threatening complications such as significant vision loss after LASIK surgery are very rare and many side effects and problems caused by laser surgery can be eliminated with re-surgery or medical treatment.
However, as with any surgery, you should be fully aware of the potential risks, side effects, and limitations of this procedure (or any other procedure). Choosing an experienced LASIK specialist can help reduce these risks and enable you to achieve the best possible results in laser eye surgery.
The first step is to determine if you are eligible for vision correction surgery. Your doctor will perform a thorough eye examination to determine if you are eligible for LASIK surgery. The shape and thickness of your cornea, refractive errors and pupil size; Eye moisture (to check for dry eye syndrome); general Health review, the latest check-up date for your eyes and any medications you are taking.
After the tests, do not worry, even if you do not qualify for LASIK. You may still be able to treat your vision with other vision correction surgeries such as PRK, LASEK or IOL lenses.

RISK FACTORS FOR LASIK EYE SURGERY AND ITS LIMITATIONS
Not all people are eligible for LASIK. Specific conditions and anatomical factors can increase the risk of an adverse outcome or limit optimal LASIK results. These include:
Excessively thin or irregular corneas
Large pupils
Severe refractive errors
Vision fluctuations
Dry eyes
Your age
Pregnancy
If there are degenerative or active autoimmune disorders.

FREQUENTLY ASKED QUESTIONS (FAQs)
Is Anesthesia Needed For LASIK Surgery?
LASIK eye surgery usually lasts less than 15 minutes and does not require general anesthesia. The laser that changes the surface of the cornea will usually be less than a minute in the eye.
General anesthesia has certain risks during any type of surgery and will significantly increase the cost of laser surgery without increasing the specific benefits. If you are anxious during this procedure, your surgeon will prescribe sedatives or other medications to help you relax before the operation. The use of sedatives is common in this practice. Most patients were amazed at how fast their LASIK surgery was performed and realized that it was easier than they had expected.

How safe and successful is LASIK surgery?
Today, with advanced LASIK devices, the safety of LASIK surgery is at a very high level. While the LASIK is very safe procedure, it can have side effects like infections or night vision problems (a halo of light that most often occurs when you see light at night, such as driving). Little percentage of people will need LASIK or another eye procedure after surgery to achieve acceptable vision.
You may also need to wear glasses when you reach the age of 40 due to the loss of near vision called presbyopia. While laser eye surgery is a highly successful procedure, it is important that all aspects of the procedure are discussed with your doctor before agreeing to surgery.

How satisfactory is LASIK surgery result?
A team of researchers at the Refractive Surgery Center in the United States and Germany analyzed the results of 97 LASIK studies published between 2008 and 2015. The results of nearly 68,000 patients who were treated were reviewed.
Based on the information gathered, the researchers found:
98.8% of patients were satisfied with LASIK surgery results.

Is LASIK permanent?
In most cases, LASIK surgery provides permanent improvement in vision. But in a limited number of cases, changes usually occur in the eye due to LASIK surgery. In some people, myopia, hyperopia, or astigmatism can recur over time and cause blurred vision.

What is the age requirement for LASIK surgery?
You must be at least 18 years old to qualify for LASIK surgery, and sometimes it is best to wait until you reach that age.

What will be the recovery time after LASIK surgery?
Your eyes will begin to heal immediately after LASIK, and initial recovery will usually begin quickly. But it is normal to experience problems such as blurred vision and fluctuations in vision for several weeks or even months after surgery.

Is my eye score right for LASIK?
During the general eye examination, your doctor will determine your eye score, whether myopia, hyperopia or astigmatism, and the score for your eyes indicates the amount of laser power needed to correct refractive errors.
Whether or not LASIK is right for you depends on several factors, such as the shape and thickness of the cornea, the surgeon, the strength and sophistication of the laser device, the type of LASIK technique used, and the parameters recommended for treatment.

Can LASIK improve myopia?
If you are over 40 years old and have trouble reading small texts without glasses or lenses, LASIK can help.
LASIK surgery can correct myopia problems caused by presbyopia with a technique called LASIK monovision.

Can I use contact lenses after laser surgery?
Although LASIK is usually performed to correct myopia, hyperopia, or astigmatism and to reduce dependence on glasses and contact lenses; however, it may be necessary to wear contact lenses or glasses after LASIK surgery.
For example, a person with very high refractive errors will not be completely cured by LASIK, and if re-LASIK is not possible, a lens may need to be used after LASIK surgery. or a person whose vision has been completely cured by LASIK, but as they get older, this person may want to use lenses instead of myopia glasses for reading.

Can LASIK cure astigmatism?
Yes, in most cases, LASIK can completely correct astigmatism and have a permanent effect on the eyes.
The ultra-precise laser in modern equipment eye surgery can be useful for shaping the cornea and treating astigmatism up to 6 points.
This highly accurate laser beam symmetries the anterior surface of the eye, thus eliminating vision problems caused by astigmatism.

Can I have LASIK if my cornea is thin?
Yes, today with the most advanced laser eye equipment even thin corneas can be treated. Operations such as LASEK, Femtolysis etc. are performed for the thin corneas.
Iranian specialized eye Clinics and hospitals enjoy advanced laser equipment, you can contact Teratona, your medical tourism facilitator in Iran, to connect you to the most reliable medical centers for your laser surgery in Iran and arrange your travel to Iran.

What are the steps in laser surgery consultation?
Before having LASIK surgery, it is essential that you have a general eye examination and advice for LASIK.
The examination is performed by your eye surgeon. The LASIK surgeon will perform the examinations and tests that are important to decide on LASIK surgery.

Which one is better LASIK or LASEK?
LASEK is a special combination of LASIK and PRK methods. LASIK is arguably the most popular laser eye surgery, but for people who are not qualified for LASIK surgery, PRK or LASEK is a better option. Like other types of laser refractive surgery, LASEK works by reshaping the cornea using an excimer laser, allowing light to reach the eye to achieve optimal visual acuity without the use of glasses or contact lenses.

Is LASIK performed on both eyes in one day?
Yes, both eyes can be operated in one session.
LASIK is a very safe surgery. The operation will take about 10 minutes in each eye. Therefore, there is usually no reason to have surgery on each eye on different days. If you want to have LASIK surgery on separate days, your surgeon will probably respect your decision; But there may be additional costs for you. Also, depending on your eye score, it may cause you discomfort if you correct one eye and operate other eye the next day.
