Mammoplasty in Iran: What You Need To Know

What is Mammoplasty?
Mammoplasty (also called Mammaplasty or mastoplasty) refers to a group of surgical procedures, the goal of which is to reshape or otherwise modify the appearance of the breast.
Types of Breast Surgery:
1-Breast Augmentation Surgery (Macromastia-Breast Hypertrophy)
2- Breast Reduction Surgery (Mammoplasty Reduction Gigantomastia)
3- Breast lift surgery (Mastopexy)

BREAST AUGMENTATION SURGERY
Breast augmentation with a breast prosthesis also known as “augmentation mammoplasty” is cosmetic surgery to increase the size of the breasts. This surgery involves placing a breast prosthesis/fat graft under the breast tissue or breast muscles to change the size, shape and texture of the breast.
Genetics have the greatest impact on shape and size of the breasts in women. Also weight changes, hormonal changes, pregnancy and lactation can have impact in shape and size of breast which make them look often asymmetric.
Breast prosthesis surgery in these cases is the most effective and immediate method of cosmetic and corrective surgery that provides a safe and secure solution and it can restore confidence in women who may need this type of cosmetic surgery.
Suitable Candidates For Breast Prosthesis Surgery
People whose breasts are fully developed.
Those who are physically and mentally healthy.
People who are dissatisfied with the size or shape of their breasts for genetic reasons or due to external factors such as aging, pregnancy or weight gain/ weight loss.
Women who have realistic expectations of breast prosthesis surgery.
Qualification of Prostheses
Prostheses should be FDA-approved and come with lifetime warranty. When you consult with your plastic surgeon for breast prosthesis surgery, you can ask about the breast prostheses used and make a decision with full knowledge and confidence.
About Breast Prosthesis Surgery
Asymmetry is the most important reason for breast surgery in women. It is possible that the size and shape of the breasts are not symmetrical due to insufficient growth of breast tissue during puberty, or after pregnancy and lactation. Regardless of the reason for breast surgery, this surgery should be performed according to the personal needs, expectations and desires of each person. Each person should consult a plastic surgeon before surgery and share their expectations with the surgeon.
The plastic surgeon will help you decide on type and size of the breast prosthesis or the amount of breast lift, the incision and sutures, and so on. Most people who decide to increase the size of their breasts are women who want their breasts to look better. Young women with small breasts are also candidates for breast prosthesis surgery. In this case, the size of the prostheses used is determined by the body shape of each person and in its most natural state.
During a breast prosthesis surgery, an incision is usually made about 4-5 cm below the breast. The prosthesis is placed in a bag under the mammary gland and under the pectoralis muscle. The plastic surgeon chooses the surgical method based on the patient’s expectations to get the best result and satisfaction in each particular case. Prostheses come in different sizes and shapes. The choice of any type of prosthesis in terms of size and shape depends on the size of the breasts, age and expectations of patient. Most breast augmentation prostheses are circular.
During the breast reconstruction process patients usually choose anatomical (teardrop) prostheses. In some cases, patients (first timers) also choose teardrop prostheses to increase the size of their breasts. The thickness of teardrop prostheses varies in the upper and lower parts and there is a risk of re-surgery because these types of prostheses are more likely to be displaced in your breast.
Suitable Size Of Breast Prosthesis
Breast augmentation surgery is a surgical procedure to increase the size and shape of the breast in women.
Obviously, the size of the prosthesis is a personal choice. However, if you ask your surgeon to do a prosthesis that does not fit your body; do not be surprised if your doctor does not do it.
Breast prosthesis sizes start at about 230 cc for a small increase or for someone who has good breast tissue can go up to 800 cc or more.
A good way to determine the right size of breast prosthesis to fit your body is to pick plastic bags and fill them with water by a measuring millilitre container. Insert the bag into your bra and see how it feels (do not use a bra that is too tight as it will not give you a precise sense of the size of the prosthesis).
Per cc = 1 ml. Therefore, 250 ml = 1 cup and 500 ml = 2 cups.
Before Breast Augmentation Surgery
Before breast augmentation surgery, make arrangements to stay in the hospital for at least 1 day and then ask someone close to you to take care of you. You will not be able to do anything for at least the first 48 hours. Teratona will arrange a nursing services at hotel for your post operative breast surgery.
During Breast Surgery
Breast augmentation surgery is usually performed at a surgical center or hospital. General anesthesia is used for this surgical procedure. Surgical incisions are made in such a way that they are not as noticeable as possible. The prosthesis is inserted through a small incision around the areola (nipple), under the armpit, in the folds under the chest. Surgery usually takes about two hours.
In this surgery, the plastic surgeon must make every effort to minimize the incision so that the scars are as invisible as possible.
After the incision is made, your breast and skin tissue is lifted, and the prosthesis is placed directly behind the breast tissue or under the chest wall muscle (pectoralis muscle). The prostheses are placed under your chest. Placing prostheses behind the chest muscle may reduce the chance of the capsule contracting (scarring around the implant makes it harder). Placement of the prosthesis behind the muscle may interfere less with mammography examination of the breast. Placing the prosthesis behind the muscle may be more painful for a few days after surgery than placing the prosthesis directly under the breast tissue. Discharge tubes are probably used for a few days after surgery and very few scars may remain.
After Breast Augmentation Surgery
The pain can be quite severe during the first 24-48 hours, although the severity varies from person to person. The plastic surgeon would prescribe you painkillers.
Have a few pillows in your bed to have a suitable support. You cannot sleep for 7 to 10 days in a position other than sitting or lying down on your back.
Sutures are used to close incisions, which may be bandaged for extra support.
Within a few days, the dressing and stitches will be removed and you may be given a surgical bra. Otherwise, you need to get the right bra (before surgery) that fits your new breast size. If your surgical incision is on the creases under your breasts, do not use spring bras with wires under them (until your breast sores have healed) as the incisions will be compressed.
During the first few days after surgery, your breasts will swell and become almost firm. Do not worry, they become softer about 4-5 days after surgery.
Some women also experience a burning sensation in their nipples for about two weeks, but it subsides as the bruising fades.
Your stitches will be removed in a week to 10 days, but your breast swelling may take three to five weeks.
Also, when removing the bandage from the incision site, you may feel that the prosthesis is too high! Do not worry about the location and condition of the breast prosthesis. They sit on your chest for the first few weeks and then gradually settle into place.
Follow your surgeon’s instruction about when to start regular exercise and activities.
Your breasts will probably be sensitive to direct stimulation for two to three weeks, so you should avoid touching too much.
Your scars will be hard and pink for at least six weeks. Then they may remain the same for several months, or even appear to have expanded. After several months, your wounds begin to fade, although they never completely disappear.
Starting around the second or third week after surgery, you should have a deep breast massage daily. This can help prevent scar tissue from forming around the prosthesis (capsule contraction). This massage should be done daily for the first six days and then weekly.
Complications of Breast Surgery
Bruising and swelling which will go away on their own over time.
Hematoma may affect the outcome of the operation and should be controlled and may require treatment. (hematoma is a bad bruise. It happens when an injury causes blood to collect and pool under the skin. The pooling blood gives the skin a spongy, rubbery, lumpy feel.)
In case of infection with swelling, redness, feeling of heat on the skin, pain and the possibility of sores or fever, visit your doctor immediately.
The body heals from all post-operative injuries and wounds by creating scars.

FREQUENTLY ASKED QUESTIONS ON BREAST PROSTHESIS

Show-More

Does breast prosthesis surgery increase a person’s risk of developing cancer?

No, according to studies it can be said that breast prosthesis surgery does not increase the risk of breast cancer, so women can easily have this cosmetic surgery without worrying about it.

Does the breast prosthesis burst?
The risk of rupture of the breast prosthesis is not permanent. In fact, it should be said that modern prostheses, implants and modern surgical techniques have made breast prosthesis one of the least dangerous and complicated cosmetic surgeries. But people who are very physically active or do sports that are very likely to hit their breasts are more at risk for their breast prosthesis to rupture. But in fact, the strength and durability of the prosthesis is not different from the other parts of body and it cannot be torn by hand pressure and impact unless an object tears the skin of the body, in which case the prosthesis is also torn.

What are breast prostheses made of?
The outer layer of all prostheses is made of silicone and the inside of these prostheses is filled with various materials such as salt water or pure silicone gel. In the past, saline-filled prostheses or silicone-filled fillings were only used for breast prosthesis surgery, but now cohesive silicone gels are more commonly used.

Do silicone prostheses harden over time?
The prosthesis may become stiff or deformed, although this may occur to 10% of patients. If this happens, another surgery will be performed to correct the problem.

Are there any complications with breast prosthesis surgery?
Breast augmentation surgery is not dangerous and does not pose a risk to patients, but this surgery like other surgeries has minor complications such as swelling, bruising, redness, etc., which resolve over time.
In some people, nipple anesthesia may occur temporarily and this surgery is less likely complicated if performed by trained and experienced surgeon.

Can I breastfeed after a breast prosthesis surgery?
Yes, but be sure to tell your doctor before the surgery. To reduce the potential risks of this surgery and its involvement in breastfeeding, the general recommendation is not to use periareolar incisions. Because after breast prosthesis and placing the implant under the muscle, there is a risk of interfering with the sensation of the nipple.

Are there any side effects for baby breastfed with a prosthetic breast?
Another question that is often asked about the side effects of breast prosthesis on breastfeeding is whether it is possible for prosthetic material to enter breast milk and enter the baby’s body. Most people think that breastfeeding with a prosthetic breast causes the contents of the prosthesis to be transmitted to the baby in this way. This idea is completely wrong. Research has shown that there is no harmful substances in the milk of women who have had a breast prosthesis surgery, and there is no difference between breast milk with breast prosthesis and others.

When can I have sex after a breast prosthesis?
Most surgeons recommend postponing sex for two weeks after the procedure. Having sex raises your blood pressure too much and can cause blood to collect around the implant and cause a complication called a hematoma. After two weeks, you should carefully perform intercourse and avoid movements that put pressure on the implant. It takes about six weeks for your married life to return to normal.

Is it possible to travel by plane for people who have had a breast prosthesis?
The idea that people with breast prostheses, due to the air pressure caused by altitude, should not travel by plane is a very wrong. Breast prostheses are not restricted to air travel, and you can easily travel after recovering from a breast prosthesis. This misconception stems from old rumors spread by foreign airlines to reduce the number of employees taking leave for breast implants.

Is it necessary to change the prostheses after a few years of surgery?
No, used prostheses have a high durability, and there is no need to replace the prostheses.

Do I need an auxiliary bra?
After the operation, you may feel discomfort and pain in your chest. As a result, your surgeon may recommend that you wear a bra during the first 2-5 weeks after surgery.

Can I take a shower as usual?
Your surgeon will give you specific instructions on how to shower and wash. You may not be able to take a shower or moisturize the treated area for several days after the operation. When you want to take a shower, you may need the help of others and you cannot do it alone.

Does breast prosthesis surgery also do a breast lift’s job?
No, if patient is also interested in breast lift, she can have prosthesis and breast lift surgery at the same time.

When can I start exercising again?
Being able to exercise depends on your physical condition. We recommend that you refrain from exercising for the first three weeks after the operation so that your body heals faster. During this time, aerobic exercise causes the breasts to swell and slow down the healing process. After the third week, you can do light activities such as cycling, and after six weeks, you can continue your previous activities.

What are postoperative cares?
Normally this surgery is performed under general anesthesia and the patient is hospitalized for one day (not overnight stay).
When the patient is discharged from the hospital, painkillers and antibiotics are prescribed. If your doctor allows, you can remove the dressing on the first day after the operation and take a bath if you wish.
Do not drive or move your hands too much in the first days after surgery
Walking is not forbidden for women who have had breast prosthesis surgery, but it is forbidden to do sports such as swimming or exercises that involve the upper torso until some time after breast prosthesis surgery.
Women who have had breast prosthesis surgery should avoid tanning for a while.
It is recommended that you avoid sleeping on your stomach for the first few weeks after a breast prosthesis.
The first examination after a breast prosthesis is usually done 3 to 5 days after the operation. In the first examination, if instructed by your doctor, a special tape (Elastic Wrap) is used, which is enclosed around the chest.
Generally, patient can return to work a few days after surgery, but strenuous activity and exercise for 2 to 3 weeks is strictly prohibited.
Most surgery swelling decreases during the first 3 to 5 weeks.
Breast tenderness, especially when touched, may last for several weeks. Post-operative examinations are usually performed in 4 to 6 weeks, 3 months, and 1 year after surgery, after which patients should be examined every few years.
What Is Breast Prosthesis Replacement Surgery?
The purpose of breast augmentation surgery is to correct the appearance of the breast or to repair it. During these procedures, the surgeon may remove silicone material from the implant and the breast capsule.
Over the years, tastes change or there are factors that make you no longer want to have puffy breasts. So no worries. You can remove your prostheses whenever you want.
Some women who have had breast prosthesis surgery may want to make the following corrections:
Removing implants and replacing them with new implants,
Relocate existing implants
Removing breast prostheses without replacing them.
What Happens During Breast Implant removal Surgery
The simple explanation is that removing a breast prosthesis is the opposite of doing a breast prosthesis. During the operation, the surgeon usually reverse engineers the method used to implant the prostheses. They usually start with the same incisions to create fewer scars. However, instead of implants, they remove them. The surgeon usually removes any scar tissue from inside the breasts.
After removing the implants and scar tissue, the surgeon closes the incisions and the patient goes to recovery. Due to its difficulty, this operation may take up to an hour.
Do not worry about scarring after surgery. Plastic surgical incisions are made in unnoticeable areas to minimize scarring. In addition, if the incision is well sutured and cared for, the scar will disappear over time.
Can I travel by plane after breast augmentation surgery?
Yes! There are no restrictions on traveling by plane after a breast prosthesis, and it is better not to pay attention to the rumors.
What Is Breast Reconstruction Surgery?
Breast reconstruction surgery to increase breast size is a great choice for those who want to restore the original appearance of their breasts, resize the prosthesis, or reshape it.
You may be a good candidate for breast reconstruction surgery based on the following factors:
Discharge or replacement of prosthesis (e.g. reduction of prosthesis volume).
X-ray indicates a leak or tear in the prosthesis.
Change the size or shape of the prosthesis
Hardening of the scar tissue around the breast area
Misplace of the prostheses from their original position
Deformation of breast tissue as a result of weight gain or weight loss
If you have experienced any of the above, you are a good candidate for breast reconstruction surgery.
For further consultation and guidance, you can contact Teratona medical tourism consultants and ask your breast surgery inquiry, price quote, and treatment travel arrangement to Iran. We will help you connect the most qualified and board certified surgeons for your breast surgery at the most affordable price.
Breast Reduction Surgery:
Having large breasts seems ideal for some individuals; however, some women with very large breasts may experience discomfort which is medically called “macromastia” complication.
Breast reduction surgery is necessary if the size of the breasts causes physical problems such as back pain, neck pain, shoulder pain or emotional distress, or one breast is larger than the other is. In this procedure, cosmetic surgeon removes excess skin and tissue from the breast area and as a result, the breasts become smaller, lighter and lifted.
How is breast reduction done?
Breast reduction is a relatively difficult surgery that is usually performed under general anesthesia. Excess fat and breast skin and glandular tissue are removed during surgery. The nipple is placed higher and the scattering of the areola (the brown area around the nipple) is reduced. The amount of tissue removed depends on the size of the breasts and the proportion of the breast size to the person’s body.
Breast Reduction by Lipomatic Surgery
The breast is made of glandular tissue and fat and in young women most of the tissue can be hard glandular tissue. As people get older and the effect of hormones, caused by menopause, on breast change and percentage of adipose tissue increases.
In older women who are overweight, the volume and weight of the breasts can be reduced with lipomatic surgery. Lipomatic surgery is usually less invasive than breast reduction surgery. The duration of the operation is shorter and it can be performed under local anesthesia with sedatives and painkillers without general anesthesia. This can be helpful if patient is old and does not want to use general anesthesia. This surgery has fewer risks and complications. The skin will not be removed during this surgery, but it will be stretched in such a way that breasts appear less sagging. Breast reduction surgery and breast lipomatic are also common in men.
Breast Lift Surgery (Mammopexy or Mastopexy):
You may be happy with the size of your breasts, but the effects of aging or the effects of pregnancy and childbirth may cause sagging breasts. Breast lift surgery removes sagging breasts without resizing them.
Mastopexy, or breast lift, is a more complex surgical procedure than breast augmentation surgery (involving only a breast prosthesis). In some cases, breast lift and breast prosthesis can be done together to achieve the desired end result.
What Is The Purpose Of Breast Lift Surgery:
Each patient is unique and the surgery is performed according to your specific needs and goals. But in general, the goal of breast lift surgery can be the following.
Eliminating sagging breasts
Restoration of breast shape
Pulling up the nipple
Reducing the size of the areola
Improvement
Mastopexy may be performed while maintaining the size of your breasts. It can also be done with a reduction or increase in size using fat injection or breast prosthesis.
In your consultation with your doctor, as a candidate for breast surgery, the size and shape of the breasts, the quality of the skin and the location of the nipples and areola will be carefully examined.
If you are overweight, your plastic surgeon may suggest that you stabilize your weight before surgery.
Breast Lift Surgery And Its Benefits:
Breast lift, also called mastopexy in medical term, is a cosmetic plastic surgery performed to rejuvenate breasts that have sagging appearance.
This popular surgical procedure removes excess skin from the breast and tightens the surrounding tissue to create a contoured, younger appearance of the breast. If the areola is enlarged (dark areas around the nipple), a breast lift can also reduce the size of the areola.
Pregnancy, breastfeeding, weight loss and having large, heavy breasts can all accelerate the aging process of the breasts and cause them to sag too early or too much. Some women are eager to have this surgery and get their breasts back to normal. In most cases, breast lift surgery also changes the position of the nipple (areola) to improve the new appearance of the breasts.
Breast Augmentation and Breast Lift Surgery:
Breast lift surgery is not performed to increase breast size. In most cases, only the unnecessary skin is removed and not the breast tissue. So, your breasts will look fuller and more beautiful as a result. Some women want to increase the size of their breasts with a lift. Therefore, in some cases, a combination of breast prosthesis and breast lift may be performed to achieve the desired results.
Postpartum Breast Lift Surgery!
The term “mommy makeover” is used to describe a set of surgeries that are usually performed after childbirth to rejuvenate a woman’s body including breast lift surgery. Breast lift is a common surgery for mothers after pregnancy.
Breast Lift/ Mastectomy
A breast lift or mastectomy is a procedure performed on women who feel that their breasts are sagging. The cause of sagging breasts is the loss of skin elasticity.
There are several reasons for sagging breast:
The mammary gland enlarges due to hormonal changes during pregnancy.
Obesity and weight gain lead to increased fat concentration in the breasts, while diet reduces fat and leaves skin loose.
Changes in age and gravity cause the breasts to sag more.
There are two ways to solve this problem
The plastic surgeon may remove the excess skin and place the remaining skin on the tissue and fat. A breast prosthesis may also be used to insert the breasts (in people with small, drooping breasts).
In some cases, a combination of the two is recommended. Depending on the surgical procedure, a surgical scar may appear around the nipple and under the breast.
Breast lift is a relatively invasive surgery and has a lot in common with the breast reduction method.
During breast surgery, part of the mammary gland with surrounding adipose tissue and skin may be removed. The nipples are protected by blood vessels around the breasts. When part of the breast tissue is removed, the remaining part is sutured. The plastic surgeon performs the surgery depending on the surgical method, the amount of tissue removed and the patient’s request in terms of breast size and firmness.
Different methods of breast surgery have a variety of effects on the ability to breastfeed. Problems may result from the removal of parts of the mammary glands and ducts. But if only the extra skin is removed during breast lift surgery, breastfeeding will not be affected. It should be also noted that the outcome of this surgery is highly dependent on the surgeon’s mastery.
