Types of Retinal Detachment

Exudative: Uncommon and infrequent detachment due to abnormal secretion of fluid between the retina and underlying tissues, which is mostly due to inflammation and bleeding of the eye.
What is retinal holes (retinal tear)?
The formation of holes in retina is often due to a sudden stretching of part of the retina. As we age, the vitreous portion of the eye loses its strength, thus weakening its connection to retina.
In this condition, the posterior parts of the vitreous are separated from retina. This condition is called posterior vitreous detachment (PVD), which is a very common condition and mostly occurs in elderly people.
This detachment is not dangerous in itself and will only cause floaters (a moving shadow in the visual field), but sometimes a tear or hole occurs when the vitreous separates from the retina.
Retinal Detachment Surgery:
Surgery is the only way to treat the retinal detachment, and the patient should see an ophthalmologist immediately for operation because medication will not be able to treat this problem.
Types of Retinal Detachment Surgery 
Depending on the type and severity of retinal detachment, different treatments are introduced for retinal detachment:
Laser or Cryopreservation of the Retina: (Laser Retinopexy)
This method is suitable for the treatment and closure of retinal perforations, but is not sufficient to resolve the problem of retinal detachment. Using a cryotherapy pen as a freezing device (from the outside of the eye) around the retinal opening, burns are created that lead to tissue adhesion and closure of the retinal opening and the establishment of stable connections between the retina and the eye wall, which it prevents fluid enters the back of the retina.
Pneumatic Retinopexy (Retinal Attachment Using Gas)
When retinal detachment occurs in the upper part of the eyeball, the gas injection method can be used. In this method, after local anesthesia, gas bubbles are injected into the eye, which leads to pressure from the inside to retina, retains and holds it in place, and it closes the retina hole and preventing fluid from leaking into the back of the retina. The gas bubble will usually be absorbed within 10 days to two weeks, after this time, around the retina hole is usually burned with a laser or cryotherapy pen to prevent the hole from coming back.
Scleral buckle Surgery:
In this process, a buckle (sponge or soft silicone tape) is sutured to the sclera (sclera or whites of the eye) and pushes the outer wall inward, bringing it closer to the detached retina so retina reconnects to its place.
For better and faster attachment of the retina to the outer wall, the fluid collected behind the retina is drained during surgery, and the retinal holes are closed using a laser or cryotherapy pen.
Vitrectomy Surgery:
When the number and size of holes or the extent of retinal detachment is large or intraocular bleeding occurs, simpler surgical procedures do not work properly, vitrectomy surgery should be used.
In this method, first the vitreous is removed from the eye with a special device and in case of intraocular bleeding, blood is washed and removed. Sterile serum (BSS) is then injected into the eye instead of the vitreous, and the retinal holes are closed with a laser.
Sometimes a scleral buckle method is used at the same time as vitrectomy to help external pressure reattach the retina.
In more acute cases after vitrectomy, silicone oil is injected to fill the eyeball from the inside and hold the retina in place with internal pressure. Due to the severe disorders, that silicone oil causes in the eyesight, after a few months in which the retina sticks in place, with a surgery, extra silicone oil is removed and sterile serum (BSS) is injected instead.
Postoperative Care
After the surgery, especially scleral buckle and vitrectomy, the eyes are painful and sensitive for several days. It is usually necessary to use antibiotic drops for up to a week or two and anti-inflammatory drops for a few weeks after surgery. Sometimes other drops (such as antihypertensive or pupil dilators) are also used. If a gas bubble is injected into the eye, the person should keep his head straight for a week or two, even sleep in a semi-sitting position, and should not sleep on his back in any way.
In general, after retinal surgery, it is necessary to see an ophthalmologist regularly to control the condition of the retina and prevent possible complications.
Retinal Recovery
Recovery and treatment of the retina depends a lot on the recovery period. In most cases, the retina will stick in place within a week or two, but in cases where the duration of retinal detachment has been long, after recovery, the patient does not have good vision. During recovery, the patient’s vision should always be monitored. After surgery, anti-inflammatory drops and antibiotics are prescribed for two weeks, during which the patient’s condition should be monitored by an eye specialist.
Care After 
After retinal surgery, the following series of cares must be taken.
After surgery, a plastic shield is placed over the eye for one to two weeks to protect and prevent trauma to the eye. This protector is washable and can be washed daily and put on the eye again after drying.
Note that after the surgery, how the patient sleeps is very important, depending on the type of operation. It is recommended that the patient sleep on his stomach. Of course, if the forehead and face are down, it is enough and the patient does not have to sleep on the abdomen.
